Insurances Accepted and Office Consultation Policies:
Please always double check with your insurance to make sure Dr. Bahar is in your network. We are unable to verify coverage for patients.
In-Network PPOs: We are in-network with the following standard PPO insurance plans:
- Anthem Blue Cross or if you have a referral through the Industry Health Network (for Motion Picture Industry & Television Fund, Writer’s Guild of America, Screen Actors Guild, Directors Guild of America)

Please remember that it is ultimately your responsibility to determine you or your child’s coverage. For patients who see Dr. Bahars and are indeed not considered “in network,” they or/their custodian will still be required to pay the full “insured responsibility amount” as described in the explanation of benefits (EOB) statement they receive.
o Out of Network Consultation Policy: We take the full consultation fees at the time of the visit. Please call our office to inquire about out-of-network rates. We do submit all claims to your insurance and you may receive a reimbursement depending on your insurance plan benefits and whether the deductible has been met for the year.
HMOs: We are contracted with the following HMO medical groups:
o Before scheduling an initial consultation, all HMO patients must have an approved referral from their pediatrician or primary care provider within their medical group. We will be asking for referral numbers at the time of scheduling an appointment.
Cash Patients: We do accept patients who have no medical insurance. Please call our office to inquire about rates.
